Professional Liability Claim Costs for Physical Therapy Providers Surge by Over 20%, New Report Finds

The financial and legal landscape for rehabilitation professionals is growing increasingly perilous, according to a comprehensive new industry analysis. Professional liability claim costs for physical therapy professionals have escalated significantly, driven by mounting exposures related to manual therapy techniques, patient monitoring protocols, complex injuries, practice-level claims, and licensure defense matters.

The average total incurred for professional liability claims involving physical therapy professionals has increased by 20.3% to reach $162,174. Furthermore, the severity of these legal actions is on the upward trajectory; the share of claims closing in the substantial $250,000 to $749,999 indemnity range rose from 9.3% to 13.7% when compared against the previous dataset. These revealing metrics are highlighted in the newly released Physical Therapy Professional Liability Claim Report: 5th Edition.

The report is published collaboratively by Healthcare Providers Service Organization (HPSO), which operates as part of the Healthcare division of Aon Affinity—the affinity business of Aon plc (NYSE: AON)—and CNA (NYSE: CNA). By analyzing recent trends in professional liability exposures, claim drivers, and licensure defense challenges, the comprehensive document aims to equip physical therapy practices, physical therapists, and physical therapist assistants with vital risk management insights, real-world case studies, and in-depth expert analysis.

Industry leaders point out that while the profession remains deeply dedicated to patient recovery and functional independence, the structural realities of modern clinical practice introduce complex vulnerabilities.

"Physical therapy professionals are committed to delivering safe, effective care that helps patients function at their best; however, they practice in an environment marked by increasing complexity and evolving risks," said Jennifer Flynn, vice president of risk management for Healthcare Providers Service Organization. "This report is designed to provide physical therapists, physical therapist assistants, and practice owners with insights and strategies to help minimize their unique professional liability exposures."

The analysis underscores that liability risks do not exist in a vacuum; they stem from the intricate daily interactions between practitioners and patients, where high patient volumes, advanced therapeutic modalities, and rigorous documentation standards intersect. As clinical expectations rise, the margin for error narrows, making administrative and operational oversights more costly than ever before.

Experts emphasize that awareness is the first line of defense against costly litigation. Understanding the precise pathways through which claims originate empowers clinical teams to adopt preventative measures before minor misunderstandings or procedural complications escalate into formal legal challenges.

"By understanding the factors that lead to claims, physical therapy professionals and practices can take proactive steps to help reduce risk and lower the chance of litigation," said Crystal Miller, RPLU, CPCU, underwriting director at CNA. "This report aims to raise awareness of trends and claim drivers to enhance risk management knowledge and support safer care for physical therapy professionals and their patients."

To support these proactive risk reduction efforts, HPSO has announced plans to roll out a specialized educational initiative in the coming months. The organization will release a series of seven distinct Physical Therapist Spotlights. These targeted releases will feature direct links to valuable resources highlighting risk control best practices across critical operational areas, including meticulous documentation standards, strategies for effective licensure defense, and the safe integration of emerging technologies within clinical settings.

The Physical Therapy Professional Liability Claim Report: 5th Edition represents an ongoing, long-term commitment by CNA and HPSO to the continuous education and professional protection of the physical therapy community. To formulate the general analysis presented in the 2026 dataset, researchers examined a sample size of 497 closed professional liability claims. This data pool encompassed physical therapists, physical therapist assistants, physical therapy practices, and various other healthcare practitioners who provided professional services as employees of an insured physical therapy practice.

The evaluated claims were officially closed between January 1, 2020, and December 31, 2025, and were specifically limited to those that resulted in an indemnity payment of $10,000 or greater, subject to the specific report limitations and considerations outlined in the complete publication.

Physical therapy professionals, practice owners, and risk management specialists seeking a deeper understanding of these evolving liabilities can access the full findings, data breakdowns, and analytical case studies. The complete Physical Therapy Professional Liability Claim Report: 5th Edition is available for download directly through the Healthcare Providers Service Organization website at https://www.hpso.com/ptclaimreport.
